Introduction:

MedSurvey, Inc. (“MedSurvey,” “we,” or “us”), a medical market research firm, takes great pride in the relationships it has built and continues to build with the medical community. We respect the privacy of each community member and survey participant thus we would like to share with you just exactly how and why we collect and process the information you submit via our website and mobile app. In sharing our policies and commitments, we hope to make all individuals using our website and mobile app (“visitors”) comfortable with the community registration process and understand the reasons why we collect a variety of information.

MedSurvey (“community”), located at https://www.medsurvey.com/sites/main, is a double opt-in healthcare community for healthcare professionals and patients. The community is solely owned and operated by MedSurvey, a market research firm specializing in conducting surveys with healthcare professionals and patients. Individuals who have completed the registration process and successfully activated their account (““member,” or “user”) are profiled by a variety of demographic information for the purpose of receiving survey invitations that pertain to their specific area of medical knowledge and/or expertise. We invite you to read through the rest of the privacy policy to learn more about how your information will be used. How registration information is collected and used

MedSurvey collects a range of information through the registration process depending on the classification of each member. The information collected throughout each step of the registration process is described below. We have also included a brief summary on why we are collecting this information and how it will be used:

a. Login Information – During the registration process, visitors are asked to create a password, which, along with their e-mail address, will serve as secure login information for their MedSurvey profile. If a member forgets his/her password, a password reset tool is available using a reminder question and answer session that a member had setup during the registration process. Member passwords are encrypted at all times and are not accessible to MedSurvey shareholders, officers, directors, employees and/or other agents.

b. Contact Information – MedSurvey collects a variety of contact information (i.e. phone number, fax number, e-mail, etc.) for the purpose of alerting members to relevant survey opportunities, disbursing payments for completed surveys (using the provided mailing address), and for other purposes relating to the member account (i.e. account update reminders, compensation confirmation, opt-in newsletters, etc.). Any personally identifiable information collected through registration is not shared with third parties other than for the sole purpose of identity verification. (see “Sharing information with third parties” below).

c. Profile Information – MedSurvey collects information from members about their job, lifestyle, and medical condition(s). This information is gathered and primarily used for the purpose of delivering relevant survey opportunities to our members. In some cases, this information may also be appended to a survey participant's survey data record and analyzed in aggregate form. This information will not be personally identifiable when shared with third parties without the participant's express consent.

d. Information for verification – In an effort to prevent fraud, MedSurvey will ask some members to provide information that would help verify their identity (i.e. professional license number). This information may be shared with a third party solely for the purpose of verifying your identity. After verification, the third party will properly discard such information and it will be encrypted and stored securely on MedSurvey servers until the member account is deleted. MedSurvey will not ask for social security numbers, credit/debit card numbers, or bank account numbers to verify a member's identity.

e. Referral Information - The “Invite your colleagues” section in the registration process and member profile is an optional feature that allows a member to invite his/her colleague(s) to join the MedSurvey community. If the member should choose to refer a colleague, the colleague's full name and e-mail address will be required. This information will only be used for the purpose of sending a one-time e-mail invitation and will not be retained in any capacity. MedSurvey reserves the right to identify the referring member in the invitation e-mail.

Information collected through surveys

Since the purpose of MedSurvey is to allow individuals to participate in paid medical research surveys, we believe it is important to explain how the survey information will be used.

All information gathered through surveys is reported in aggregate form only and will not be directly associated with any personally identifiable information. If a survey sponsor requests the identity of survey participants for any reason, MedSurvey will contact the respondents directly in order to receive express permission for the release of such information.

In the event that a survey sponsor requests validation of survey participants for a particular project, a third party will be contracted to contact a random sample of respondents who participated in that specific survey to verify some of the provided information. The contracted third party will only use personally identifiable information to contact some of the respondents and will properly discard all such information after the end of such a project.

After qualifying for a survey, the respondent will be asked to provide a mailing address that will be used to process the survey payment after the respondent successfully completes the entire survey.

In some cases, the survey respondents will have the ability to audio record their response to a question. In this case, a third party will have access to the audio recording but the recording will not be associated with any personally identifiable information, unless provided by the survey respondent in the content of the recording.

Information relating to survey payments

Any compensation offered for completing a survey will be clearly stated on each survey invitation. In an effort to properly disburse survey payments, MedSurvey will ask each survey participant to enter and confirm the mailing address where he/she would like to receive the payment. This information will be used to mail survey payments.

Info Information about income earned from surveys and survey participation history will be stored securely on our servers and will be offered to members via their secure online profile. Some of this information might be used for marketing purposes, but only in aggregate form without personal, individually identifiable information (i.e. total survey payments paid out during the course of a year).

Passively collected information

MedSurvey will collect certain information using a visitor's browser to produce statistics on how our website and mobile app are being used. Knowing these statistics helps us improve user experience. All passively collected information will not be personally identifiable.

Testimonials

Occasionally, MedSurvey may ask a community member for a non-paid testimonial that summarizes his/her experiences with MedSurvey. If a member agrees, a consent form will be sent to the member outlining the terms of use of the provided testimonial. A signed consent form is required from the member for MedSurvey to use the testimonial in any way. Personally identifiable information (such as name, specialty, and city/state) may be used to identify the author of the testimonial. At any point, a member can request MedSurvey to stop using their testimonial by sending an email to info@medsurvey.com. The testimonial will be removed according to the conditions outlined in the original consent form.

Use of cookies

A cookie is a small text file that is stored on a visitor's computer to record and store certain data. MedSurvey uses session ID cookies to authenticate the member after initial login in order to allow easy profile browsing. The session ID cookie expires (deleted permanently) when you close your web browser. We do not link the information we store in cookies to any personally identifiable information you submit while on our site.

Control of personal profile information

Every member can use his/her unique username and password setup during the registration process to securely access the membership account from the MedSurvey home page (https://www.medsurvey.com/sites/main). Most of the fields provided during the registration process can be easily updated through this process. In an effort to maintain the most current information, MedSurvey will e-mail each member a reminder every six months to update his/her profile. 

Deleting MedSurvey account

In the event that a member wishes to delete their MedSurvey account, he/she will need to complete a request from their MedSurvey profile. This can be done by signing into the profile from the https://www.medsurvey.com/sites/main website, and clicking on the “Cancel Membership” link located under “Profile Settings.”

Security of information

MedSurvey takes certain security measures to protect any information submitted by members. A secure SSL encryption ensures the highest degree of security when data is being transferred online. Additionally, MedSurvey has certified www.medsurvey.com with the following organizations and proudly displays their seals on our home page:

www.medsurvey.com is monitored in order to maintain the highest level of data security/integrity.

The security of your personal information is important to us. When you enter sensitive information on our registration or order forms, we encrypt that information using secure socket layer technology (SSL). To learn more about SSL, follow this link.

We follow generally accepted industry standards to protect the personal information submitted to us, both during transmission and once we receive it.  No method of transmission over the Internet, or method of electronic storage, is 100% secure, however.  Therefore, while we strive to use commercially acceptable means to protect your personal information, we cannot guarantee its absolute security.

Sharing information with third parties

MedSurvey does not share any personally identifiable information with third parties without a member's express consent with the exception of validating a member's identity during signup. MedSurvey does provide certain member information in aggregate form to clients and/or vendors, but such data does not contain personal, individually identifiable information.

Law enforcement activities

We may disclose any information about members to law enforcement or other government officials as we, in our sole discretion, believe necessary or appropriate, in connection with an investigation of fraud, intellectual property infringements, or other activity that is illegal or may expose us to legal liability.

This privacy policy only pertains to the practices and commitments set forth by MedSurvey for www.medsurvey.com. We recommend that you review the privacy policy of other sites to learn about their specific policies.

Children policy

MedSurvey does not solicit its community membership to minors and only allows individuals who are eighteen (18) years of age or older to join MedSurvey. In order to join MedSurvey, the member must agree to terms of use that clearly state that a member must be at least eighteen (18) years of age to qualify for membership.

Transfer of ownership

MedSurvey reserves the right, in the evolution of its business to reorganize itself by buying or selling additional business assets. MedSurvey's business practices, including those associated with MedSurvey itself, and the personal information of its members may be transferred or sold to another business entity or individual.  While MedSurvey may take efforts to ensure that a successor entity honors these terms of use and the associated privacy policy, MedSurvey, its shareholders, officers, directors, employees and/or other agents will not be responsible for any change or failure to honor these terms by any successor

International visitors

The information submitted on www.medsurvey.com and the mobile app is stored in the United States of America. By browsing our website(s), completing the registration process, and using the mobile app, visitors understand and consent to the transfer of data to the United States of America.

Changes to the privacy policy

We reserve the right to modify this privacy policy at any time. Should there be any changes to our privacy policy, we will post the changes to this privacy statement and we will display a notification on the MedSurvey home page for 30 days.
